Our services

At Prairie Rose Family Medicine, we keep our pricing simple, transparent, and built around your needs — not insurance billing codes.

There is a one-time enrollment fee of $150 per member (maximum $300 per family), waived for the first 50 members, to cover the time, care, and behind-the-scenes effort it takes to thoroughly gather and review your medical history so we can begin our care relationship on the same page.

There is no long-term contract—just a relationship built on mutual trust and respect. If you ever need to step away from your membership, we simply ask for 30 days’ notice so we can wrap up and transition your care thoughtfully.

Rejoining later is subject to availability and may include a $300 re-enrollment fee.

Newborns (0-12 months)

From your baby’s first days through the many milestones of their first year, we are here to support your growing family with comprehensive, compassionate care. We offer routine newborn checkups, sick visits, feeding and sleep guidance, developmental screenings, and trusted advice to help you feel confident in your baby’s health and well-being

Young Children (1-6 years)

As your child begins to explore the world, we provide attentive, personalized care during this active stage of development. Our visits include routine wellness checkups, sick visits, milestone monitoring, and support for common concerns like sleep, behavior, and nutrition — all with a focus on partnership and reassurance for parents.

Children (7-13 years old)

These early childhood years bring big changes, and we are here to guide you through them. From preschool physicals, to when your child gets the sniffles, and on to monitoring behavioral and developmental support, we take the time to get to know your child and build a strong foundation for lifelong health — with care that’s friendly, accessible, and deeply personalized.

Adolescents (13-18 years old)

School-age children and teens need healthcare that grows with them. Whether it’s routine physicals, sports physicals, sick visits, mental health support, or help navigating adolescence, we build trust with your child and offer a safe space for open conversation, preventive care, and long-term health planning.

Healthcare shouldn’t be a hassle during college. Whether you are at home or away, we offer convenient care for common illnesses, stress-related concerns, mental health support, and preventive needs — with virtual and in-person options that fit into a student’s schedule and budget.

Your membership at Prairie Rose Family Medicine covers a wide range of services designed to support your health through every stage of life — with no hidden fees, rushed appointments, or surprise bills. Here’s what you can expect:

  • Annual wellness visits with basic labs, tailored to your age, health history, and risk factors

  • Chronic disease management for conditions like diabetes, high blood pressure, asthma, and more

  • Acute visits for illness and minor injuries

  • Telemedicine visits when appropriate

  • Basic office based procedures such as skin biopsies, laceration repairs, and incision/drainage

  • Routine gynecologic exams

  • Care coordination with specialists, emergency departments, and hospitals when needed

Eva’s Membership

Eva’s Membership is designed to make high-quality, personalized healthcare affordable and accessible for your whole family. This plan includes up to 2 adults and 3 children for a flat monthly rate of $450.

Have a larger family? No problem — each additional child can be added for just $15/month. We believe in supporting families of all sizes and understand that every household is unique. This plan is a great way to keep everyone in your family — from newborns to parents — connected to the same trusted care team, with no surprise fees or rushed appointments.

Investing In Your Health

Looking for a way to simplify your payments and save at the same time? When you choose to pay annually, you’ll receive 5% off your total membership cost for the year.

It’s a great option for families and individuals who prefer to take care of billing all at once while enjoying a bit of extra savings. Plus, it allows you to focus on what really matters — your health — without having to think about monthly payments.
